REQUEST MEDICAL RECORDS

Copies of medical records are provided to patients, physicians, health care providers, insurance companies, attorneys and Social Security and disability representatives.

How to Request Information
To obtain a copy of a medical record, print the Authorization for Release of Information form complete it and sign, then fax or mail to the location listed below in which the patient was located.

Requests are processed in the order in which they are received. The time it takes to complete your request can vary. Let us know if the requested information is needed by a specific date and every effort will be made to meet your needs. Federal and state laws govern release of medical information.

Fees
Under Georgia law, you may be charged a fee for administrative and copying services. These fees can change from year to year. For a current fee schedule regarding release of information, please contact the Health Information Department at 404-350-7325. Fees are waived for copies provided directly to a health care provider for continuing medical care.
