Refer Your Patient for Spine or Chronic Pain Treatment

Shepherd Center requires a provider referral to make an appointment for spine and chronic pain treatment. To refer your patient, download the Shepherd Spine and Pain Institute Provider Referral Form.

In addition to the referral form, complete the forms and send in your patient’s medical records as listed below:

  • Demographic sheet
  • Consultation notes and office visit notes
  • Diagnosis reports (X-rays, CTs, MRIs, EMGs, etc.)
  • Procedure notes
  • Operative notes
