Next Steps

Please complete the following form.

Once we have received all your information, you will be contacted to set-up an appointment with one of our Multi-Specialty Medical Clinics to be evaluated for the Day Program for Spinal Cord Injury.

If you are found to be a candidate for the Day Program, we will try our best to give you an accurate timeframe for your admission. Please note, your admission will not be scheduled immediately after your specialty appointment, and, there is a possibility that your admission will need to be delayed as unforeseen instances sometimes arise with Shepherd Center’s current acute patients.

  • Radiology tests (i.e., X-rays, MRIs, CT Scans.)
  • Office visit notes, labs and testing (from the past two years.)
  • Any other information you feel may be of assistance.

What happens after all my documents have been faxed?

  • Once we have received your records, we will contact you to schedule an appointment.

Preparing For My Appointment

  • You MUST bring MRI films/X-Rays/Radiology CDs with you to your appointment.
  • You MUST arrive at least 30 minutes prior to your appointment time.
