Is Day Program Right For You?
- Upon admission, you must be able to tolerate at least three hours of treatment daily.
- You must have realistic goals with at least two discipline areas to be considered a candidate.
- You and your caregiver must be able to be present Monday-Friday from 9:00 a.m. to 4:00 p.m.
- You do not have any medical issues that would prevent daily therapy, such as a serious wounds, unmanaged pain, or unmanaged blood pressure/cardiac issues.
- At the time of discharge, we expect you to return to the discharge environment as independent as possible in the home and the community.

Once we have received all your information, you will be contacted to set-up an appointment with one of our Multi-Specialty Medical Clinics to be evaluated for the Day Program for Spinal Cord Injury.
If you are found to be a candidate for the Day Program, we will try our best to give you an accurate timeframe for your admission. Please note, your admission will not be scheduled immediately after your specialty appointment, and, there is a possibility that your admission will need to be delayed as unforeseen instances sometimes arise with Shepherd Center’s current acute patients.
Additional Documents Needed
In order to guarantee a successful appointment, your primary care physician or specialist must fax the following medical records and forms to Admissions at (404) 603-4504:
- Original History and Physical Exam note from the time you were diagnosed or presented with symptoms. (If diagnosed many years ago, just the past two years will do.)
- Written report of the findings of your most recent
- Radiology tests (i.e., X-rays, MRIs, CT Scans.)
- Office visit notes, labs and testing (from the past two years.)
- Any other information you feel may be of assistance.
What happens after all my documents have been faxed?
- Once we have received your records, we will contact you to schedule an appointment.
Preparing For My Appointment
- You MUST bring MRI films/X-Rays/Radiology CDs with you to your appointment.
- You MUST arrive at least 30 minutes prior to your appointment time.
- If you cannot make it to your scheduled appointment, you MUST call Outpatient Services at (404) 352-2020 at least 48 hours prior to reschedule your appointment.

- At Shepherd Center, we are committed to appropriate care for our patients. Please understand that your initial appointment is a consultation only and is NOT a guarantee that further services can be provided.
- Shepherd Center physicians are unable to serve as your primary care or internal medicine physician.
Due to the volume of patients we see, it is very important that you appear for your appointments on time. Shepherd Center reserves the right to terminate future scheduling for patients who do not show up for appointments and fail to give proper notice in advance.
